The Highest Heel Enterprises, LLC. offers one FREE product catalog to each customer. Additional catalogs are available for a nominal fee of $ 5.00 each plus shipping and handling. If you wish to receive a product catalog:
Please submit the following items:
1. A copy of your resale license
2. A completed copy of the Catalog Request
Form
If you choose to pay by Credit Card, also submit:
3. A completed copy of the Credit Card Authorization

4. A copy of the front and back of your Credit Card

The Highest Heel Enterprises, LLC. offers international customers one free catalog. However, a $ 10.00 fee will be assessed for postage and handling. A credit for this charge will be issued on your first order.
The one-time charge is payable by:
- All major credit cards
- Wire Transfer
Note:Any bank charges regarding international payment methods such as wire transfers/cashier’s checks to The Highest Heel Enterprises, LLC. will be at customer’s cost.

Please submit the following items:
1. A completed copy of the Catalog Request Form
